Home Care Coordinators

 

We Provide More Than a Caregiver: Our Home Care Coordinators Are Supervisors, Educators, Communicators and Problem Solvers


Our Home Care Coordinators provide Geriatric Care Management Services, supervising our Personal Attendants in your home, providing oversight to help assure high quality care.
They meet with you to discuss your life circumstances, and your desires and wishes. Your Home Care Coordinator then develops a Plan of Care that the Personal Attendants follow. The Home Care Coordinator makes regular supervisory visits to your home to ensure quality of care and the updating of your Plan of Care as your needs change.

Along with supervision from our Home Care Coordinators, our Personal Attendants provide the proper level of support while encouraging independence as much as possible. We seek to help you maintain your health and functionality. This is different for every client, and is one reason why our Home Care Coordinators play such an important role, providing support, education and guidance to our Personal Attendants.

An essential role for our Home Care Coordinators is in reducing the potential for harm. (Click here for a deeper discussion of Harm Reduction for the Elderly.)

Our Home Care Coordinators go beyond the personal needs of our clients, and help to manage all of the issues related to maintaining a clean and safe home environment. They assist with a variety of life issues from pets to contractors, doctor’s appointments and coordination with Home Health and Hospice, reporting to attorneys and conservators, providing accountability for household expenses, and maintaining consistent communication with family members who may live far away.

Quality In-Home Care really does require more than just a caregiver, and our Home Care Coordinators are provided at no added cost. Our Home Care Coordinators are available by phone 24 hours per day, 7 days a week, so our Personal Attendants are not left to solve problems on their own, but have supervisory backup support when needed.
