Case Management


 

CASE MANAGEMENT PROGRAM

We provide Case Management for clients who are not receiving in-home care from our Personal Attendants.

For example, a client with a beginning onset of failing memory and family members who live out of town may need accompaniment to appointments with multiple medical providers, so that information is clearly received and followed. Case Management may involve coordinating with pharmacies, dental, optometrist and podiatrist appointments, obtaining medical and safety equipment for the home, and contractors, electricians, and plumbers to resolve home maintenance issues.

For adult children who live out of town, Case Management provides an added layer of consultation, service, professional oversight and protection for a parent who is becoming more frail, but who is still self-sufficient enough to not require personal care or companionship.

For attorneys, conservators, fiduciaries and trustees, Case Management can provide the professional  ”boots on the ground” to ensure your client is getting to the doctor, eating well and provide you the 24 hour crisis intervention your clients might need.

Case Management fees are included in the Personal Attendant rates when utilizing Personal Attendant Services.
